SIGNS AND SYMPTOMS OF CHOLANGITIS 

  • Fever
  • Chills
  • Fatigue
  • Yellow discolouration in the skin, mucous membrane, sclera
  • Right abdominal pain
  • Vomiting
  • Nausea
  •  Dark coloured urine
  • Clay coloured stools

CAUSES OF CHOLANGITIS 

  • Gall stones – Cholesterol stones, Pigment stones, Mixed stones
  • Primary sclerosing cholangitis
  • Narrowing of bile duct
  • Obstruction in the bile duct
  • Parasitic infection
  • Bacterial infections

PATHOPHYSIOLOGY OF CHOLANGITIS 

Cholangitis due to gall stone obstruction is more common. If there is increased cholesterol secretion and decreased bile acid secretion, supersaturation of bile occurs. This leads to the precipitation of secretions along with cholesterol in the gall bladder gradually gall stones are manifested. These gall stones obstruct the bile duct causing inflammation and associated symptoms.

  • Abdominal ultrasonography (ultrasound)
  • Computerized tomography (CT) scan
  • Endoscopic retrograde cholangiopancreatography

TREATMENTS FOR CHOLANGITIS 

  • Antibiotics to control bacterial infections
  • Removal of obstruction and draining of fluid
  • Treatment of underlying pathology
  •  Surgical interventions

PROGNOSIS OF CHOLANGITIS 

  • Prognosis is based on the cause of the bile duct inflammation and the condition of the patient

COMPLICATIONS OF CHOLANGITIS      

  • Liver diseases
  • Portal hypertension
  • Recurrent infections

  • In Ayurveda,cholangitis is managed based on the treatment principle of ‘yakrit pleehodara’.

  • Excess aggravation of doshas
  • Virudhahara – incompatible food
  • Rudhapadha – obstruction in bile duct by pittashmari

  • Kshud nasa – loss of appetite
  • Chirat pachanam – delay in digestion
  • Sahate na athisouhityam – intolerance to excess eating

  • Agnimandya and tridosha prakopa adversely affect the swedavaha, udakavaha and rasavaha srothas leading to yakrit pleeha vridhi and associated symptoms.

  • Avoid high cholesterol diet

YOGA FOR CHOLANGITIS 

  • Nadi Shuddi Pranayam    – refresh the body and mind

The patient needs to be seated in a meditative posture with the head and spine erect, and the body relaxed. The patient has to close one nostril (e.g. left nostril if using the right hand and vice versa) with the thumb and exhale completely through the other nostril. Again, he will have to breathe in deeply through the other nostril while the opposite nostril is still closed with the thumb.
