The symptoms occur when exposure to cold :

PATHOPHYSIOLOGY OF COLD URTICARIA
In cold urticaria, high levels of histamine and other chemical messengers are released in the skin when expose to the cold. These substances cause the blood vessels in the affected area of skin to open up and become leaky leading to redness and swelling.

Again, he will have to breathe in deeply through the other nostril while the opposite nostril is still closed with the thumb.
